RE: Paintball Game (the “Game”) on _______________( the “Game”)

By_____________________ (the “Group”)

IN CONSIDERATION of Adrenaline Paintball providing paintball game facilities and permitting the Group and each participant including any minors to participate in the Game I, the undersigned (the “Organizer”) hereby certify as follows:

That I am the person responsible for organizing and supervising the Group's participation in the Game and I am 19 years of age or older.

That for each participant who is under the age of 19 years (a “Child”) and is not accompanied by his or her parent or legal guardian on the Game Date, a Waiver and Release of Liability Form (a “Waiver”) duly signed by that child's parent or legal guardian has been completed, signed by that child's parent or legal guardian has been completed, signed and is attached to this certificate.

I have personally witnessed the signature of each Waiver by the child's parent or legal guardian and the identity of such parent or guardian has been proved to me by a valid drivers license bearing a true photograph of the parent or legal guardian, his or her name accurately and completely written on the signed Waiver.

I acknowledge and understand that Adrenaline Paintball is relying on the truth, completeness and accuracy of this Certificate in determining whether to permit any child to partcipate in the Game and that I will be personally responsible for any untrue, incomplete or inaccurate statement.

This certificate is given in my capacity as a representative of the Group and in my personal capacity and as such personal liability attaches by the delivery of this certificate.
